Release form ?

Hi!
Does anybody have a generic release form they are willing to share, or let me know where I can find one? It would (obviously) be for shamanic work, in my case taking clients on a shamanic journey. Some people seem to start experimenting on their own, and I want to make sure I’m legally safe.
